Care Coordinator

Our Care Coordinator is part of our healthcare team and works to ensure that patients receive seamless care. PMC patients who are seen in the emergency department or discharged from the hospital are contacted by the Care Coordinator to discuss changes in medication, referrals to specialists and to ensure you are recovering well from your illness. Patients may call the Care Coordinator with questions or to find out what community resources are available.The Care Coordinator, in conjunction with providers and nursing staff, works with patients with chronic or complex health conditions to develop a plan for comprehensive care. The Care Coordinator strives to
help patients achieve the best care possible.

Angie Barnes has been a nurse for 16 years with inpatient experience in Medical/Surgical, Pediatrics, Cardiac Telemetry, Post-partum and Labor and Delivery care. Angie has worked at PMC for 10 years providing direct nursing care, triage and care coordination. Angie educates our patients with diabetes through one-on-one visits and group classes. She is part of the Diabetes Self-Management Education Program at Physicians’ Medical Center, which provides quality comprehensive diabetes self-management education and support to patients with diabetes, empowering them to achieve their health goals.

Wellness Coordinator

At PMC, we are reorganizing the way we provide the Medicare Wellness physicals. Medicare does not cover a physical (except for the initial Welcome to Medicare physical that has to be done within first 12 months on Medicare) but will cover a Medicare Wellness Screening Visit. As a patient you will still be able to see your provider for acute/chronic problems and exams.

Medicare Wellness screenings will be performed by our Wellness nurse coordinator, yearly. They will guide you through the full range of preventive services offered to help keep you healthy and find problems areas early, when treatment is most effective.

It is always your choice to accept or decline any options presented to you. Again, it is not a visit to replace you seeing your provider, it is just a prevention based service that is free of charge to you as the patient. The visit is paid for 100% by Medicare yearly; however there are some labs and immunizations that are recommended but Medicare may not cover. Through this new program we hope to provide you with the resources you need to take advantage of what your insurance offers and help you feel more in charge of your health care.

Pain Care Program

a behavioral health team member is a licensed psychologist who works with chronic pain patients to help them improve their quality of life. Her work focuses on helping patients establish lifestyle change and new coping strategies. You can meet with her individually and as a part of pain school.
